Physiotherapy or wait it out? How to know when to book
Not every ache needs a physio, but waiting too long doesn't help either. Here are clear signs to know when it's worth booking and what to expect.
You treat the knot, feel better for a few days and it returns. If that sounds familiar, the problem isn't the knot: it's what causes it. We explain how to tackle the root.

Your neck and trapezius feel like a rock. You get a massage, loosen up for a few days… and two weeks later it’s right back. If you’ve fallen into that loop, you’re not alone: it’s one of the most common reasons for a visit. And it’s almost always the same story: the knot gets treated, but not what creates it.
A muscle knot is a muscle that has stayed in tension and can’t relax. Easing it with heat or massage feels good, but it’s like lowering a fever without checking why it went up: if the cause is still there, the muscle knots up again.
And in the neck and shoulders, that cause is usually in three places:
A one-off massage releases the area, and it’s fine as part of the treatment. But if it’s all you do, you enter a cycle: knot → relief → relapse → knot. Each time you go back sooner and each time the effect lasts less.
Breaking that cycle takes two things at once: easing the area now and, above all, changing what overloads it.
In the clinic we approach it step by step:
We look at how you move your neck, what your posture is like and which areas are compensating. A trapezius knot often comes from lower down (mid-back) or from the jaw.
With manual therapy and, when it’s indicated and you agree, dry needling on the points that trigger it most. That gives you room to work.
We prescribe specific guidance for your daily life (how to set up your screen, micro-breaks, how to lower your shoulders) and simple strength work so your neck holds up without knotting again.
The goal isn’t to make you depend on the couch, but for you to understand your neck and know how to keep it well on your own.
If you’ve spent months in the loop of a knot that comes and goes, if the pain rises to your head or if some days it limits your work or sleep, it’s worth looking at it calmly instead of keeping on covering the symptom.
At FisioSentis we’ve spent over 15 years treating necks and backs in Zaragoza. We start by listening to your case, explain what triggers it and build a plan so that this time it doesn’t come back.
